Axler says the rankings are used to help identify gaps in service and address barriers to health care. Access to good doctors, education level, a well-paying job, and your housing situation are all factors that could play into your health.

This year, the analysis shows that meaningful health gaps persist not only by place but also by race and ethnicity.

In Ottawa County, the rate of obesity is at 29 percent, excessive drinking at 21 percent and 67 percent of women are getting their yearly mammograms. Holmes, Coahoma and Quitman were ranked as the least healthy counties. The project includes maps on its websites that lets users compare counties for each measure.

The 2018 health rankings show the St. Cloud area slightly improved.

The authors of the report card noted that children in poverty are less likely to have access to quality schools, and have fewer chances to be prepared for higher-wage jobs.

The top five counties for health outcomes, according to the report, are: Ozaukee (No. 1); Taylor (No. 2); Pepin (No. 3); Washington (No. 4) and Waukesha (No. 5).

The information is helpful to anyone interested in improving health in their community, Jerome said.

Other southeastern Wisconsin counties lagged in the rankings.

But despite higher spending, the US has the lowest life expectancy and highest infant mortality rate than any other wealthy nations.

"All counties experience ups and downs".

"We can't be a healthy, thriving nation if we continue to leave entire communities and populations behind", said Dr. Richard Besser, RWJF president and CEO.
