Updated Colorectal Cancer Screenings Guideline Lowers Age

The American Cancer Society (ACS) announced Wednesday, March 30, that it has updated its guideline for colorectal cancer screening, changing the age people at average risk should begin screening from 50 to 45. Previously, the recommended age to begin screenings is 50, but increased rates of cancer and death in those younger than 50 have encouraged health care providers to revise the starting age.

The updated guidelines come in response to rising colorectal cancer rates and deaths in younger adults.

Despite the high cure rate when colon cancer is caught early, only one-third of the Americans over 50 get screened.

"But as we saw data pointing to a persistent trend of increasing colorectal cancer incidence in younger adults, including American Cancer Society research that indicated this effect would carry forward with increasing age, we chose to reevaluate the age to initiate screening in all USA adults".

The guideline suggests screening either through a high sensitivity stool-based test or visual examination such as colonoscopy that snakes a tiny camera through the rectum to view the colon to search for abnormal tissue growth.

For colorectal cancer, the American Cancer Society didn't push one screening option over another but listed various options: High-sensitivity stool tests, created to detect blood in feces, which need to be administered every year; a DNA stool test, sold under the brand name Cologuard, every three years; a colonoscopy, every 10 years; or a virtual colonoscopy or flexible sigmoidoscopy, every five years.

What explains the rise in colorectal cancer?

"It is the only one of those tests that actually prevents cancer. It is well established that the risk of CRC has been increasing dramatically in young adults (under age 50) over the last 15 years but the new ACS-commissioned analyses were the first to take race, gender, and this increased risk into account".

"For the past, maybe as much as ten years, we're seeing younger and younger patients with advanced colon cancer", O'Neil said.

The same tests are recommended by the U.S. Preventive Services Task Force, a panel that reviews evidence and issues advice for a variety of screenings and treatments.

The qualified endorsement of screening at age 45 is reasonable and "will lead to a lot of discussion and investigation", said Douglas Rex, a professor of medicine at Indiana University. And they typically do not show the traditional risk factors - obesity, smoking, physical inactivity or high-fat diets.

Breedlove said she's heartened by the change but believes the guideline should go even lower.

"I think it's great that they are finally recognizing that colorectal cancer is being seen at younger ages and it's not any longer just being considered as the older man's disease", Georges said. The study then compared screening strategies starting at ages 40, 45 and 50 and stopping at ages 75, 80 and 85.

In a young person, Cercek noted, gastrointestinal symptoms likely stem from an infection or other non-cancerous condition.

Colonoscopy also carries risks, including bowel perforation and complications from anesthesia.
