Corrections officers did not check on Epstein for "several" hours prior to his body being discovered, according to the Washington Post.

Epstein, who was found dead in a NY prison cell on Saturday in an apparent suicide, was also alone in his room when he died, according to a senior prison union official, despite procedures that he should have been accompanied by another inmate.

Dr Sampson said that a city medical examiner conducted the examination on Sunday while a private pathologist observed, at the request of Epstein's representatives.

It wasn't clear what the substitute's regular job was, but federal prisons facing shortages of fully trained guards have resorted to having other types of support staff fill in for correctional officers, including clerical workers and teachers.

The person said that the Metropolitan Correctional Center's Special Housing Unit was staffed with one guard working a fifth straight day of overtime and another who was working mandatory overtime.

"If it wasn't Mr Epstein, it would have been somebody else, because of the conditions at that institution", she told the newspaper.

A law enforcement source also told the Times he was alone in his cell Saturday night after his cellmate was transferred. "It was inevitable. Our staff is severely overworked".

"(Legal) proceedings are not decided on by the government", she said, noting that since 2013 it is forbidden in France for a justice minister to give prosecutors instructions relating to individual investigations.

The guards failed to follow several protocols leading up to Epstein's death, according to the New York Times.

The incident was quickly met with suspicion as Epstein is linked to many famous and powerful people and had inexplicably been taken off of suicide watch, despite reportedly trying to kill himself just weeks earlier.

Ms Giuffre, who now lives in Queensland, said she was angry that he would not face the courts after their years of struggle to be heard.

What happens to the case against him?

The details of how Jeffrey Epstein died in his Manhattan jail cell over the weekend have yet to be released, but medical officials have performed an autopsy on the high-profile inmate accused of sexually abusing underage girls.

Epstein had been charged with one count of sex trafficking of minors and one count of conspiracy to commit sex trafficking of minors.

The Federal Bureau of Prisons, which operates the MCC, provided no explanation beyond its terse statement that Epstein was found dead in an apparent suicide.

Attorney General William Barr, calling for an investigation by the FBI and the Justice Department's inspector general's office, said he was "appalled" to learn of Epstein's death while in federal custody.

Barr also issued a stern warning, saying the case was far from over and that anyone who may have conspired with Epstein "should not rest easy".

Barr, the top USA law enforcement official, vowed to carry on the case against anyone who was complicit with Epstein. An additional federal investigation was launched Saturday after the Federal Bureau of Prison said Epstein was found unresponsive in his cell at a high-security jail in Manhattan.

Born and raised in New York, Epstein worked as a maths and physics teacher in the mid 1970s before moving into finance, creating his own firm: J Epstein and Co.
